Na sdělení vážné diagnózy by pacient neměl být sám

„To se mi vůbec nelíbí, pošleme vás na okres“. S takovou větou se na začátku své cesty, která končí často v onkologickém centru, setkávají pacienti u svých praktických lékařů poměrně často. Podobně na tom byla i do té doby naprosto zdravá Jana Pelouchová. K lékařce se vypravila kvůli únavě a slabosti, které se stupňovaly. „Přičítala jsem je jaru, jenže když mi paní doktorka prohmatávala břicho, výraz jejího obličeje mě vylekal,“ vypráví Jana Pelouchová.
„Později jsem zjistila, že jedním z příznaků mé choroby - chronické myeloidní leukémie (CML) - je vedle enormního počtu bílých krvinek i zvětšená slezina.“
Výsledky krevních testů upozornily lékařku, že má před sebou pacientku s onemocněním krve, následovala tedy další vyšetření u obvodního hematologa. S potvrzením diagnózy CML se ocitla Jana Pelouchová konečně ve specializovaném centru, kde jí začali léčit.
Svou diagnózu se tedy nedozvěděla najednou, ale po částech. „Lékaři často zapomínají na to, že pacient vnímá ohrožení vážnou nemocí již od prvního okamžiku,“ říká Libuše Kalvodová, psycholožka Interní hematoonkologické kliniky Brno a předsedkyně Psychoonkologické sekce České onkologické společnosti. „Navíc se nejistota nemocného oddalováním jasného sdělení jen zesiluje.“
Jana Pelouchová říká, že se šokem ze sdělení "máte rakovinu" se musí každý nakonec stejně vypořádat sám, ale že by od lékaře například ocenila, kdyby jí dal naději: "Lékař by měl změkčit toto sdělení například tím, že pacientovi řekne o nových lécích, které léčbu podstatně zlepšují, říká pozdější zakladatelka pacientského sdružení DIAGÓZA CML. "V prvním okamžiku navíc informace člověka úplně paralyzuje, není moc schopen vnímat. Proto je dobré podpořit oznámení choroby brožurou a kontaktem na pacientskou organizaci, dodává.
Pravdu potřebuje především nemocný
Dřívější zvyk nesdělovat diagnózu nemocnému, ale jen jeho blízkým, přinášel řadu nevýhod. Vedle nedokonalé spolupráce nemocného s lékaři si stejně většina pacientů uvědomovala, že situace je vážná. „Pacienti chtějí a potřebují slyšet, s čím mohou při léčbě počítat. Na našem oddělení se jen velmi zřídka stává, že by nemocný pravdu slyšet nechtěl,“ říká docent Edgar Faber z Hemato-onkologické kliniky v Olomouci. Vedle sdělení diagnózy a možností léčby je ještě třeba nechat prostor pro dotazy, i když je to někdy technicky obtížné. „Když nemocný otevřeně vyjadřuje své obavy, klade otázky, mnohem lépe pak svou léčbu zvládá,“ dodává Faber.
Na brněnské hematoonkologické klinice je zvykem, že sdělení vážné diagnózy nemocný nemusí čelit sám. Rozhovoru se kromě lékaře, sestry a psycholžky mohou účastnit i rodinní příslušníci, které si pacient sám vybere. Délka rozhovoru zcela závisí na nemocném. Nejdříve mu lékař vysvětlí nemoc a léčbu, sestra režim a pak se pacientovi a jeho rodině věnuje psycholog. Podle potřeby je pak možné takové setkání opakovat.
Nemocnému nejlépe rozumí nemocný
Ucelená péče nejen o tělesnou schránku, ale také o psychiku nemocného, je zatím spíše výjimečná.
